Insurers review these reports when preparing a quote to confirm the information on your application and to understand your background as a driver. Your auto insurance history report is what gives you a clear picture of your past as an insured driver. It pulls together notes from older policies, showing:
When your coverage began
Any changes you made along the way
When it ended
Whether there were gaps between policies

Whether a policy was cancelled, and why
Any claims you filed
In Canada, these reports usually come from:

How do I order an auto insurance history report?
You can order your own Consumer AutoPlus™ report or DASH Consumer Driver report for free once per year. Here’s how to order an auto insurance history report for car insurance:

Visit the CGI website and download the Consumer AutoPlus™ Request Form, or the IBC website and download the DASH Consumer Driver Report Request form.

Fill out the form online or by hand and double-check that all required fields are completed.
Make a clear copy of both sides of your driver’s licence

Make a copy of a document that confirms your address, such as a pink slip or utility bill.

Submit your completed form and identification documents either by email or mail.
Keep in mind that because digital submission is faster, emailing your request means you’ll likely receive your report sooner.

To submit your request to CGI Inc., you can send your completed form and supporting documents either by email or by mail. Once CGI receives everything, they’ll process it and mail your report to the address shown on your identification documents, and most people receive it within about 10 business days. Submission for IBC:
To submit your request to the Insurance Bureau of Canada (IBC), you can send your completed form and documents either by email or by mail. If you choose email, make sure your attachments are password-protected and that you send the password in a separate message for security. Here are the steps:

Email your password-protected form and documents to DASHConsumerRequests@ibc.ca and send the password in a separate email, or

Mail your package to the IBC (DASH Consumer Request) office in Toronto.
Email requests are usually processed within 10 days, and your report will be emailed back to you. Mailed requests take about 10 to 20 business days, and the report will arrive by regular mail via Canada Post.
myAutoPlus app

Once your identity is confirmed, you can have immediate access to your report whenever you want. Consumer AutoPlus™ report
If you spot something that doesn’t look right on your AutoPlus™ report, CGI says to contact the Complaint Officer or Ombudsperson at the insurer that added the information. They’re the ones who can look into it and request a correction. If the issue isn’t fixed after speaking with them, you can dispute the entry under the Consumer Reporting Act, or reach out to the Registrar of Government and Consumer Services, who can direct CGI to adjust the report or file a complaint.
DASH Consumer Driver report
If you notice an error on your DASH report, you can dispute it by emailing IBC at DASHConsumerRequests@ibc.ca to request the DASH Consumer Driver Report Correction form. Once you fill it out, you can send it back to IBC with your supporting documents by email or mail.
