When a broker inputs your driver's license number, the request generates your report and delivers it to the insurance company or brokerage.

  • Visit the site to complete the CGI Consumer Autoplus™ Request Form
  • Towards the bottom of the form, you will be directed to either scan and email the signed form to insurance.helpdesk@cgi.com or mail it to:
    • CGI Technical Assistance Centre
      5800 St-Denis, 7th floor
      Montreal, PQ, H2S 3L5
      By signing the form, you are requesting your own information. If there are any details that belong to another driver in your report, CGI may require their consent. You should get the report by mail within 10 to 14 days.

If there are errors, you can either contact the company, or apply to the Registrar of the Ministry of Government and Consumer Services to direct CGI to make corrections and/or file a complaint.
